Pull the skin tight. Veins only roll cause you're not holding traction properly.
Here is the big one. When you see a flash, don't get excited. The needle is in but the catheter probably isn't. Advance a little more. Remember: the needle is longer than the catheter.

And for a bonus, start distal (hand) and work up. Save the AC for last ditch, trauma, and adenosine.
